How At HOME Works
All patients begin with the Annual Access membership, which provides priority scheduling, an ongoing monthly newsletter, and secure record management. This annual membership fee is administrative and is required to establish care. After enrolling, you may choose to add one of our additional at HOME care plans: either the Basic Prepaid Care Plan or the Comprehensive Primary Care Plan. Once you are registered, our office will contact you to complete enrollment of your selected membership plan.
Annual Access Membership: Best for occasional care users. This is required for all plans.
Basic Prepaid Care Plan (Add-on): Best for frequent users to benefit from discounted rates.
Comprehensive Prepaid Care Plan (Concierge Add-on): Best for those wanting full concierge at HOME care.
